Thank you for volunteering to participate in the Illinois State University Institutional Artifact Portfolio process for Assessing the General Education Program.

Upon receipt of this form, a University Assessment Office Staff Member will e-mail you to: 1) confirm the pick up of your artifacts of student work and 2) request a copy of the assignment instructions or your syllabus. Please note that the artifacts will be copied and returned to your department's/school's central office within 1 business day.
